I guess Nancy Hughes may have been recast in 1956. Irna Phillips was not happy with Helen Wagner and sent Nancy on vacation after 13 weeks.but the decision was made to retain Wagner and she resumed the role.

OLTL considered recasting Will Rappaport when Jason Shane Scott left, but decided to bring in Al Holden instead.

In 1998, there were rumors about AMC potentially bring in a Kendall recast when they began casting for a mid-20s female whose description sounded familiar to Kendall. In the end there was no Kendall recast then and the character ended up becoming Kit Fisher.

When Jensen Buchanan left AW in 1994, they were going to recast, with Kelly Rutherford and Ellen Wheeler (former Vicky/Marley) testing for the roles. Instead they gave Jensen the moon and the stars to return.

Y&R made a production of trying to find recasts for Jill once Jess Walton had issues with the show in 2009.

I don't know if this counts, but around the time Joan Collins had joined GL as Alex there was some serious consideration into recasting Roger with Charles Keating being named as a possibility.
