I thought for years they were going that way, even when Sarah was still at GH. While it doesn't make sense, I thought GH could be sick enough to go in that direction.

I saw that way back when... she was so protective of him when SB first came on to play that role and the vibes between her and Anthony always screamed molestation which led me to the conclusion that Johnny was her son not her brother... kinda like Spyder Games except the dad was dead there. Eh.
