I think the clinic-looking scenes are a dream/hallucination. He's only back for a day! I would be extremely surprised if are they going to have him revealed to be alive and then have him die later in the episode.

As for the picture of Leo in a suit at Confusion, I wonder if maybe that is (or was intended, before being picked up by PP) for the finale. LB thinks ahead. Remember she had Cady shoot that extra scene for Palmer's memorial when she was in LA shooting the stuff for JR's cancer story. Maybe the plan was for Greenlee to catch a glimpse of an in-fact alive Leo in the final episode, or have the audience see an alive Leo watching Rylee.

Josh said his storyline was respectful to where Greenlee is now so it's not unrealistic for Leo to be revealed to be alive (recently recovered from his long coma) and have him walk away from Greenlee. But if he is dead and Budig decides to jump ship, at least they've laid the foundation for them reuniting in heaven.
